Notes:
Note            a single paragraph note
NB              start of a multi-paragraph note
NE              end of a multi-paragraph note
Miscellaneous:
E               example text (i.e. fixed width)
V               verbatim text (i.e. fixed width, ignore escapes)
E80             wide example text
Line            horizontal line
N               normal text
NV              name-value pair (i.e. hanging indent)
PB              page break
Sign            signature for a memo, etc.
