    * Teach ToTerm.pm to get terminal width RT#85467
      Get a terminal width and pass it to Pod::Text::Termcap
      from one of these sources (in order):
          1. An explicit width set from command line with -w
          2. MANWIDTH environment variable
          3. stty output
          4. The default width of 76 (same as Pod::Text)
