Changes for version 1.1 - 2014-07-13

Added support for custom fields in create_issue(). Changed to allow multiple potential transitions in transition_issue(). Made error reporting more informative. Added new trace() method for even more debugging. Refactored all requests to use a single internal request method. Extended and unified handling of update hashes. Formatting fixes in POD.
