Changes for version 1.00 - 2007-09-21

Add new "gsafd" database to test.xml. This is an OCLC SRU server that correctly implements result-set IDs and RSID searching.
Otherwise identical to v0.94 (which was really the last of the 1.0-prerelease versions). With this version, the software is ready for public release.
