Test calling conventions of continuation().

Changes for version 0.04_01 - 2016-01-28

Major code refactoring.
Amend documentation slightly.
Validation of arguments is now handled by Params::Validate.
Use the "boolean" pragma for true/false values.
Do no longer recommend Test::Pod and Test::Pod::Coverage.
Adjust indentation and omit diagnostic for test file.
